Crossroads Supportive Housing
Crossroads offers case management, support services, and supportive housing for homeless individuals and families. This facility is located in Sandusky, Ohio.
Serenity House Supportive Housing
Serenity House provides transitional housing for homeless chemically dependent adults who desire to participate in treatment and recovery programs. This program has two components including group living and independent scattered site apartments and is located in Sandusky, Ohio.
Homeward Bound
This is an alternative to emergency shelter and supportive housing for households at risk of becoming homeless by moving them directly into permanent housing and providing individualized home-based supportive services.
Homeless Prevention and Rapid Re-Housing (HPRP)
Assistance is provided to individuals or families to mitigate homelessness through supportive services and rental assistance. The ability to sustain permanent housing thereby ending the cycle of homelessness is an important element of this program.
